This new user-friendly document (link here) called “Netivot,” which has been reviewed by Rabbi Hershel Schachter shlit”a and Rabbi Mordechai Willig shlit”a, contains two parts: 1) A healthcare proxy & directive, in which a person appoints their proxy and rav and declares that they want their decisions to be done in accordance with Jewish law. This is a legally binding document in all 50 states and Ontario, and will soon also be available in Quebec, the UK, and Australia.

2) A conversation guide for people to express their general preferences regarding end-of-life care decision making.

This is a non-legal document which is halachically-sensitive and meant to generate healthy conversations within families.

Rabbi Weiner explains the new features of these documents and offers a pastoral guide on how to help people effectively utilize these forms, particularly in the hospital setting.
